Starting July 1, 2025, CMS expands the OASIS (Outcome & Assessment Information Set) requirement to every home-health patient in your care—whether they’re on Medicare, Medicaid, private pay, or any other non-Medicare plan.
Collect and Submit Mandatory OASIS Data
Collect and submit OASIS data for all patients with any pay source who are not exempt and who begin receiving home health care with an OASIS SOC (M0090) date on or after July 1, 2025.
Include all subsequent OASIS time-point assessments: resumption of care, recertification, other follow-up,
transfer, discharge, and death at home.
Who's Affected?
All patients 18 & older with a SOC (M0090) date on or after July 1, 2025.
All payers: Medicare, Medicaid, private-pay, VA, etc.
Includes all subsequent OASIS time-points: ROC, Recert, Transfer, Discharge, Death-at-home
Exemptions: Patients under 18, maternity-only, personal care–only services.
